Providence Community Resource Representative - Forest View in Anchorage, Alaska
Description
Under the direction of the Care Management leadership team, will work collaboratively with the Social Service Clinicians/Social Workers (SW), RN Care Managers (CM-RN), Utilization Review RN's (UR-RN) and/or other business partners in denial management by performing multiple support service duties that facilitate a seamless transition of patients throughout the care continuum using current knowledge of inpatient and outpatient processes and community services. This position requires frequent contact with patients and families including but not limited to hospital staff, community resource agencies and payer representatives. This position requires the individual to take initiative and be proficient in prioritization skills, organizational skills, communication skills and a commitment to quality and excellence in service.

Required qualifications:
Bachelor's Degree in Social Work, sociology, psychology, gerontology, human services or other liberal arts/social sciences related field. All positions placed in long-term care settings must meet this requirement. All positions placed in outpatient or acute care settings may substitute experience.
4 years Relevant work experience as a Care Management Coordinator (I) or in social services or healthcare setting.

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
