Providence Clinical Pharmacist in Mission Hills, California

Description

Under the direction of the Clinical Pharmacy Manager, the ambulatory care clinical pharmacist works collaboratively with medical group physicians and allied health providers, staff, operations and leadership to improve patient outcomes through appropriate medication and disease management (including but not limited to diabetes, hypertension, hyperlipidemia, anticoagulation). Provides coordinated, comprehensive, high-quality care to Facey patients. Responsible for the evaluation, interpretation and management of patient's medication therapies to improve the quality and outcomes of healthcare provided. Supports clinicians and other health care providers through interdisciplinary team in-service education, collaborative drug therapy management services and medication conversion programs to ensure that the use of pharmaceutical agents are rational and cost-effective. Guides department staff for optimal delivery of department services and precepts Pharmacy students as assigned.

Direct Patient Care/Population Health:

Performance Criteria

  • Responsible for providing collaborative drug therapy management (CDTM) in designated Facey services in-person, telephonically, and by telehealth.

  • Performs pharmaceutical assessment and reassessment of patient medication regimen including reconciliation and adherence to patients in the ambulatory setting.

  • Acts as a consultative resource to physicians and other members of the health care team to assess patient’s medications for interactions, duplications, contraindications, potential formulary coverage issues, and cost effective alternatives, and makes recommendations to the prescribing provider.

  • Coordinates the pharmaceutical care of patients including the initial pharmaceutical assessment and reassessments of ambulatory patients.

  • Develops appropriate care plans that are tailored to the patient’s needs. Upon request, executes these care plans through comprehensive patient monitoring and documents changes in the patient’s therapy or condition in the electronic medical record.

  • Analyzes lab results and other clinical data and provides appropriate clinical monitoring. Orders appropriate laboratory tests necessary for monitoring outcomes of medication therapy according to established protocols or in consultation with the primary care physician.

  • Partners collaboratively with the care team to enhance patient care delivery

  • Supports advancement and growth of Pharmacy programs by applying innovative medication management models

  • Exercises judgment on conditions requiring consultation, referral or evaluation by patient’s PCP or other health care professional.

  • Assists in the teaching of health care providers, patients and their family members/caregivers about the patient’s medications, directions for use, and other information as deemed necessary. Identifies barriers to patient compliance and assists patient in overcoming barriers.

  • Utilizes pharmacy data, registries and other reports to develop and implement strategies to improve patient pharmacotherapy regimens.

  • Implements and monitors drug conversion programs and drug savings programs

  • Participates in the quality assurance and quality improvement of Pharmacy Programs

  • Participates in Facey’s Pharmacy and Therapeutics Committee (P&T); and other committees or meetings relevant to the functions of pharmacy services.

  • Uses, protects, and discloses Facey’s patients protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.

Educator/Preceptor:

Performance Criteria

  • Provides education to Facey providers, staff and patients in written and verbal formats that include formal and informal presentations.

  • Assists in clinical instruction and supervision of medical assistants, certified pharmacy technicians, pharmacy students and others as required.

  • Participates in collaborative disease state education programs to improve the quality and outcomes to our patients

  • Serves as a drug information resource to Facey

  • Participates in company-wide in-services and clinician presentations including CME supported activities.

Successfully completes Environment of Care modules in the required timeframes and participates in fire drills and emergency exercises. May label, transport and store hazardous waste as described in annual training.

Required Qualifications:

  • Graduate from an accredited school of pharmacy.

  • PharmD.

  • California Pharmacist Registered upon hire.

  • Three (3) years experience with direct patient contact in acute or ambulatory care setting.

Preferred Qualifications:

  • Residency in ambulatory or managed care programs.

  • Chronic disease state knowledge base and related pharmaceutical counseling and/or motivational interviewing experience.

  • EHR experience (interpreting and inputting data).
