Providence Clinical Program Coordinator RN - Heart Clinic - Advanced Heart Failure in Portland, Oregon
Description
The RN Coordinator position is responsible for team collaboration and care delivery in a comprehensive heart disease program, internally and externally. The RN Coord Clinic coordinates multiples aspects of the heart program including new patient contact, appropriate screening evaluation and maintains communication through the initial phase of treatment. The RN Coordinator will coordinate and work closely with the RN Coordinator Program to ensure seamless patient care delivery through the continuum of care.
The RN Coordinator position will be a liaison with referring providers and hospitals that are part of the Providence heart program, and will coordinate and ensure that all are satisfied with all components. Through collaboration with key clinical and administrative leaders at Providence, the Coordinator ensures that the managers and Providers are aware of patient progression.

Required Qualifications:
Bachelor's Degree in Nursing. Graduate from an accredited school.
Upon hire: Oregon Registered Nurse License.
Upon hire: National Provider BLS - American Heart Association.
5 years Clinical experience in acute care, cardiac ambulatory and/or cardiac interventions.
Preferred Qualifications:
- Master's Degree in Nursing.
