Providence Senior Clinical Pharmacist - Medication Assistance Program in Wilsonville, Oregon
Description
This is a part-time, temporary role covering our Wilsonville and Portland locations, as well as some work from home capabilities.
The Senior Clinical Pharmacist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients. The responsibilities of the clinical pharmacist will predominantly fall into the following areas:
(1) Patient Care - The Senior Clinical Pharmacist is responsible for providing collaborative practice services in collaboration with the healthcare team.
(2) Population Management - The Senior Clinical Pharmacist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.

Required Qualifications:
Bachelor's Degree PHARMACY from an accredited School of Pharmacy. OR
PharmD PHARMACY from an accredited School of Pharmacy.
Coursework/Training Postgraduate Year One (PGY-1) Pharmacy Residency. OR equivalent educ/experience.
Upon hire: Oregon Pharmacist License.
Upon hire: Pharmacy preceptor license in applicable state as applicable.
3 years Pharmacy practice experience with at least 2 years of related experience in lieu of Postgraduate Year One (PGY-1) Pharmacy Residency
Preferred Qualifications:
- Within 2 years of hire: Board Certified by the Board of Pharmacy Specialties specific to area of practice within 2 years of starting the role.
